Pet Week Documentary

Thank you for considering being a part of this documentary. Please review the below requirements before submitting your questionnaire.

1. Must be available for 2 filming days (one documentary photo / video day and one video interview day) before the deadline of April 21.
2. Full releases must be signed by all adult participants and both parents/guardians of minor participants allowing for full social media and marketing use of all materials created.
3. Must currently have the pet to be featured.
4. Must be willing to openly share about your journey with featured pet.
